Assessment:

 

Comprehensive clinical services to older adults and their families.


 Evaluation and assessment of physical, functional, and social abilities of older adults.

 
Offer counseling and emotional support

Intervention:

  Identifying services available to the older adult experiencing lifestyle adjustment including: loss, retirement, relocation and the aging process.  Health issues (physical and emotional) including: stress, anxiety and depression.


Education and counseling to alleviate the strain and stress experienced by adult children, caregivers and family members.


Alzheimer's disease and other dementias, Parkinson's Disease, Multiple Sclerosis, Cardiac, Stroke, Cancer, Diabetes, Orthopedic and other illnesses.

 

Resources:

 

  Knowledge of a wide range of services and community resources including housing alternatives (nursing home and assisted living), rehabilitation programs, hospice and home care alternatives.

  Provide consumer education and advocacy
  Referrals to Elder Care Attorneys, Physical, Speech and Occupational therapists and support groups.

  On-site workshops and lectures available for large and small groups individualized to your needs
